NOTE: A joining fee of SGD2,500 per team is required. This amount will be fully donated to UNICEF.

Donation instructions:

The joining fees should be made directly to UNICEF, the beneficiary of this event. Please write your cheque to "UNICEF East Asia and Pacific Regional Office".
Kindly bring your cheque to Choa Chu Kang Sports Hall during the Platts Futsal Challenge on 10 March 2012 and hand it to our Platts staff during registration.
Alternatively, you can mail your cheque to Platts — 30 Cecil Street #13-00 Prudential Tower Singapore 049712, attention to Carolyn Tan.

By registering to participate in the Platts Futsal Challenge, I agree to abide by the following terms and conditions.

All players must be currently under gainful employment in the energy sector.
The Platts Futsal Challenge enables members of the energy industry to participate in a friendly sport to promote social networking. As such, players who are members of professional leagues will not be allowed to participate.
Due to time constraint, a maximum of 20 teams will be accepted, subject to a first-come, first-served basis.
